Question Description
This assessment provides an opportunity for you to evaluate an existing care coordination program as the initial step in instituting practice changes that can positively influence health and wellness outcomes.
By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:
- Competency 1: Analyze clinical priorities for a specific population that can influence health outcomes in the care coordination process.
- Explain how the care coordination process aligns with the continuum of health care.
- Describe the methodology which supports the financial, ethical, legal, social support, and provider components of the care continuum.
- Competency 3: Develop an effective interprofessional collaboration strategy for improving population health outcomes at all levels.
- Explain how interprofessional collaboration among various disciplines can facilitate the achievement of health goals for patients, the community, or a population.
- Competency 4: Propose a comprehensive plan of care for a patient or a population that includes the essential components of care, such as patient engagement, cost efficiency, data accuracy and confidentiality, outcomes, and interprofessional collaboration.
- Evaluate the cross-disciplinary support that contributes to evidence-based practices within an organization.
- Competency 5: Communicate effectively with diverse audiences, in an appropriate form and style, consistent with applicable organizational, professional, and scholarly standards.
- Write clearly and concisely, using correct grammar and mechanics.
- Support main points, claims, and conclusions with relevant and credible evidence, formatting citations and references using current APA style.

- A title page and references page. An abstract is not required.
- A running head on all pages.
- Appropriate section headings.
- Your evaluation report should be 4–5 (not to exceed 5) pages in length, not including the title page and references page.
- Explain how the care coordination process supports the continuum of health care.
- Provide evidence to support your assertions and conclusions.
- Describe the methodology that supports the financial, ethical, legal, social support, and provider components of the care continuum.
- Evaluate the cross-disciplinary support that contributes to evidence-based practices within an organization.
- Describe the level of support provided by current practices for the organization’s mission, vision, and values.
- Consider the policy and regulatory guidelines and standards that should determine current practice.
- Explain how interprofessional collaboration among various disciplines can facilitate the achievement of health goals for patients, the community, or a population.
- What evidence do you have to support your assertions and conclusions?
- Write clearly and concisely, using correct grammar and mechanics.
- Express your main points and conclusions coherently.
- Proofread your writing to minimize errors that could distract readers and make it more difficult for them to focus on the substance of your evaluation.
- Support main points, claims, and conclusions with relevant and credible evidence, correctly formatting citations and references using current APA style.
